Community Well-Being Agent with Stroud / Cotswold PCN Social Prescribing Team

Permanent position working 18 hours per week spread over Wednesday, Thursday, and Friday.
£19,000, rising to £20,000 per annum pro rata after a successful six month probation period.

Are you a caring, outgoing person who loves to work one-to-one with people to help them get the most out of life?  Can you network locally to build up a detailed picture of what groups and services there are in your area, so that you can make connections for people to help with their well-being with non-medical interventions?

Independence Trust has an exciting opportunity for a Community Well-Being Agent to come and join our Stroud team, covering the Stroud / Cotswold PCN area, as part of a friendly group of six staff who provide the Community Well-Being Service (Social Prescribing).

You don't need any specific qualifications for this role.  If we think you have the right personality and potential for this job, then we'll train and support you.  Some of the qualities that we're looking for include being passionate and enthusiastic about supporting people, a 'can-do' attitude, a flexible approach to working in a team, the ability to travel locally, and some experience working or volunteering with vulnerable individuals.
