Salary £23,600.85 per annum (pro rata)
18hrs per week
Stroud & Cotswold Area Primary Care Network (Part of Stroud / Berkeley Vale Locality)

Independence Trust have an exciting Social Prescriber post working in the Community Wellbeing Service within Stroud and Berkeley Vale Locality.  You will be working with a team of Community Wellbeing Agents to provide a service to those who may have low health needs, be in vulnerable groups, have chronic illness, well managed long-term conditions, or mild depression or anxiety.

You will work collaboratively within GP clusters and have regular communication with GP Practices whilst developing strong, sustainable relationships with Integrated Community Care Teams, Community Hospitals, Community Groups and Voluntary Sector Organisations including the Community Hubs.

The role will involve taking responsibility for a caseload of clients and the line management of the Community Wellbeing Agent assigned to your cluster.  You will handle sensitive and confidential information and complete a full suite of data.  There will be the opportunity to undertake 1:1 work with individuals and to support the development of personal support plans.  You will contribute toward key outcomes, including supporting the reduction of frequent attender appointments at health care settings.

We are looking for enthusiastic and passionate workers who possess excellent partnership working skills.  You need to have previous experience of working in Adult Health, Adult Social Care or Community settings and have a relevant qualification or experience to undertake the role successfully.  Excellent communication skills and a knowledge of the locality is essential.  You will need a broad understanding of the statutory Health and Social Care provision within the county as well as knowledge of the services provided by the Voluntary Sector within Stroud and Berkeley Vale locality.
