Armed Forces Social Prescribing Link Worker
Have you served in the Armed Forces? Are you interested in supporting ex-forces personnel with their health and wellbeing across Gloucestershire?
We are currently recruiting for a Social Prescribing Link Worker for Armed Forces. The main purpose of the role is to help develop a Social Prescribing service for ex-forces personnel, their families and carers across the county. As part of the Stroud and Berkeley Vale Community Wellbeing Service you will undertake assessments of need and help to develop a robust and holistic service for individuals. You will achieve this through finding and working directly with ex Armed Forces personnel, ensuring that they are receiving the full support they need through appropriate medical care, connecting them to local community resources and identifying skills that could be utilised in engaging in activities or facilitating events for other personnel. You will also build strong relationships with partner organisations including Armed Forces specialist support, statutory services, Primary Care and Voluntary & Community Sector partners and the Department for Work and Pensions (DWP).
